Complete the form, print it, and mail it to us along with a check for $10 and a self-addressed, stamped envelope. A copy of the page(s) requested will be mailed to you. If the name appears in more than one news article, all will be sent to you for a single $10 fee. If there are multiple articles, be sure you give all references in your request.
