Postal Request Form

Complete the form online, print it, and mail it to us at the address on the form along with a check for $10 and a self-addressed stamped envelope. For faster turnaround, use the Buy Now button below to make a secure online payment using your major credit card, then follow the Email Request Form link. Fill out the email request form and, when you click the Submit button, it will be emailed to us. Once your online payment and email form have been received, our librarian will make a copy of the obituary abstract and email it to you as an attachment. It will be sent as a .jpg (picture) file. When you receive this .jpg file, click on it and it will be loaded into your picture viewer from which you can print a hard copy by clicking on File and then on Print.
